Childhood chronic Diseases significantly affect millions of children's lives. These groups of patients often require multiple medications to treat their conditions and therefore are at an increased risk of medication related problems. This project will contribute to the knowledge base on medication related problems – its magnitude, risk factors for occurrence, severity, preventability, and also investigates its impact on health related quality of life of children with chronic diseases. It will help to identify challenges; opportunities and enabling factors which help us design effective pharmaceutical care programs. The research design will be a six month hospital based prospective observational study. The study center is Jimma University Specialized Teaching Hospital, in Ethiopia, at both inpatient and outpatient settings. Study Participants will be children aged 0 to 18 years and with a diagnosis of any of the following epilepsy, asthma, chronic heart Failure, or type 1diabetes.
