Standardized Care Plans in Community Health Care

The main objective of this study is to develop knowledge about how the use of Standardized Care Plans (SCPs) based on the International Classification of Nursing Practice (ICNP®) influence patient safety and continuity of care in community health care. Patient safety and continuity of care will be exemplified by using pressure ulcers (PU) and malnutrition as cases. Specifically, I will report on: A: how SCPs are being used for patients in risk of/or treated for malnutrition and/or PU, and the congruence between the nursing documentation with SCPs and the nursing care given to these patients in the clinical setting. B: how nurses evaluate SCPs in terms of usability, usefulness, influence on nursing practice, and continuity of care. C: how SCPs influence the quality of nursing documentation in relation to measures against malnutrition and PU.
